Y If you have a deductible you will be required to pay in full at each
visit or until we can verify that your deductible has been met.
Y You are required to pay your co-pay each visit or your appointment
will be rescheduled.

Prescriptions cannot be called in after-hours, weekends or
holidays, as we need your chart to verify dose and medications. Please
plan ahead for your prescriptions.

All lab results will be discussed during
your office Only

Filling out forms – There is a $35
charge for FMLA, Disability, or any other form. We require 5 business days to
complete any form. Some employers will reimburse you this
charge. Effective Jan. 1, 2003.

Please call 24 hours in advance to cancel or reschedule your
appointment. Otherwise a $50 NoShow fee will apply.

If your call requires that you speak with Dr. Bourne or Dr. Bauer
you will be required to schedule an appointment.
Y Please inform us of changes to your address, phone and insurance.
Y If you do not provide us with the proper insurance information you
will be responsible for the billed amount.
Y Prescriptions, labs slips or authorizations require 24 hour notice
to be completed.
